Universal Security Instruments Model MI3050S Alarm Registration Form

If your Universal Security Instruments battery powered alarm has been correctly identified as affected by this recall, please fill out the form below. If you choose to use our photo upload process, be sure to have the product on hand before starting the registration process.

Did The Alarm Sound When You Pressed The Test Button?

If the alarm sounded, no further action is required. Please remember to test your alarm weekly, and clean it monthly. If the alarm did not sound, please continue to the next step to send us a photo of the back of your alarm.

Model Number & Manufacture Date Location:

The model number and manufacture date will be on the back of your alarm. Please enter the model number and date, exactly how they appear on the back of your alarm, into the fields on the form.

Send us a photo of the back of the alarm

Provide an image of the back of your alarm to verify that it is affected. Please follow the steps below.
  1. Write your name, address and contact information on a sheet of paper, and place it next to your alarm.
  2. On a neutral background, place your alarm back side up, so the product label is facing the camera.
  3. Center your alarm in the photo.
  4. Remove all other objects from view.
  5. Take a clear, well-lit picture.

MI3050S Recall Info:

Universal Security Instruments smoke alarm safety alert only affects battery powered Model MI3050S, manufactured between January 19, 2015 through July 11, 2016.

Hazard:

It has been reported that some alarms may have been installed without proper activation and testing. If you press the test button on your alarm and it sounds, your alarm is operating correctly and does not need to be replaced.

Remedy:

Correctly identified alarms will be replaced.
