IMPORTANT RECALL INFORMAITON

SUMMARY:

Universal Security Instruments smoke and fire alarm important recall information for Models MPC322S & MPC122S. Only alarms with specific manufacturer's date codes are affected.

  • Model MPC322S - Manufacture Date = 2017JUN09
  • Model MPC122S - Manufacture Date = 2017JUN02
HAZARD:

The alarms can fail to alert consumers to the presence of a hazardous level of carbon monoxide, posing a risk of carbon monoxide poisoning or death. Carbon monoxide (CO) is an odorless, colorless, poisonous gas.

REMEDY:

Correctly identified alarms will be replaced. Follow steps below to determine if your alarm is affected.

INCIDENTS & INJURIES:

Universal Security Instruments has received two reports of units that failed to alarm for the presence of carbon monoxide within the specified time requirement. No injuries have been reported.

IDENTIFICATION:

To determine if your alarm is affected, follow the below steps. (Jump to MPC322S Guide) / (Jump to MPC122S Guide)

1 COMPARE YOUR ALARM to the product image to the rightbelow. Affected alarms are identical to the image with the red alert symbol.
Verify your smoke alarm has Universal Security Instruments Photoelectric Smoke & Fire + Carbon Monoxide Alarm printed on the cover.

2 LOCATE THESE MARKINGS ON THE BACK OF YOUR ALARM.
Remove the alarm from the mounting bracket by twisting it counter clockwise.

3 If your alarm has now been correctly identified as model number MPC322S with a manufacture date of 2017JUN09, it may be affected. Please continue to the Product Registration form for a replacement.
START MPC322S ALARM REGISTRATION
4 IF YOUR ALARM IS NOT AFFECTED - no further action is needed. Reattach the alarm to the mounting bracket by rotating it clockwise. Test the alarm by pressing the test button. The horn will beep 3 times with a 6 second pause and then beeps 4 times. The red smoke LED blinks in sync with the 3 beeps. The red CO LED blinks in sync with the 4 beeps. Please remember to test your alarm weekly.

CAUTION: If your alarm is hardwired, this means wired in directly to the house wiring: IMPORT ANT SAFETY WARNING! Before removing the alarm from the mounting bracket, turn off the AC power at the circuit breaker or fuse box. The green LED will now be off. Do not proceed with the product inspection if the green LED is on. Remove alarm from the mounting bracket by rotating It counterclockwise. Contact USI immediately with any questions 1-877-220-0046.

START MPC122S ALARM REGISTRATION
4 IF YOUR ALARM IS NOT AFFECTED - no further action is needed. Reattach the alarm to the mounting bracket by rotating it clockwise. Turn on the AC power at the circuit breaker or fuse box. The green LED will now be on. Test the alarm by pressing the test button. The alarm will sound 3 beeps, followed by a 6 second pause, then 4 beeps. The green LED will be on and the red smoke LED will blink in sync with the 3 beeps and the red CO LED will blink in sync with the 4 beeps. Please remember to test your alarm weekly.

MPC122S & MPC322S Recall Frequently Asked Questions

1. Why is Universal Security Instruments taking this action?

Universal Security Instruments, Inc., in cooperation with the U.S. Consumer Product Safety Commission, is recalling photoelectric combination smoke and carbon monoxide alarms manufactured in 2017. The alarms can fail to alert consumers to the presence of a hazardous level of carbon monoxide, posing a risk of carbon monoxide poisoning or death. Carbon monoxide (CO) is an odorless, colorless, poisonous gas.


2. How do you identify recalled units?

The cover of the alarm must say Photoelectric Smoke & Fire + Carbon Monoxide Alarm. If the alarm says Universal Security Instruments, Inc. on the cover it is battery powered. If the alarm says USI Electric on the cover it is an AC/DC model which requires the power to be turned off prior to removing from the bracket. After you turn off your AC power at the circuit breaker or fuse box, the green LED will now be off and then it is ok to remove the alarm from the bracket. If green LED is still on DO NOT remove from bracket. Contact USI immediately with any questions at 1-877-220-0046.

If you have a model with the correct markings on the cover remove the alarm from the mounting bracket by twisting it counter clockwise. The models covered by the recall are models MPC122S with a date code of 2017JUN02 and MPC322S with a date code of 2017JUN09. After checking your alarm, please reinstall and push the test button to make sure your unit is working properly.

4. What do I do while waiting for a replacement?

If you have a recalled alarm, reinstall the alarm while waiting for a replacement.


5. What do I do after I receive my replacement?

Once you get the replacement, follow the installation instructions outlined in the product user’s manual. Be sure to test the alarm. Deface your recalled alarm with the word “RECALLED” on both the front and back of the alarm in permanent marker, send a photo of the defaced unit to USI at safety@universalsecurity.com, and then dispose of the recalled alarm in your household trash.
